The University Health Service requires incoming students to complete the Health History and Immunization Form and the Allergy and Medication Form prior to the start of classes via UHSConnect. Students will need to register for\u00a0<a href=\"https:\/\/www.rochester.edu\/uhs\/uhsconnect\/\" target=\"_blank\" rel=\"noopener\"><strong>UHSConnect<\/strong><\/a>, our online patient portal, to complete the required forms.<\/p>\n<p><a href=\"https:\/\/www.rochester.edu\/uhs\/uhsconnect\/\" target=\"_blank\" rel=\"noopener\"><strong>Click here<\/strong><\/a>\u00a0for directions on how to register for UHSConnect to complete your forms. Please complete and submit the forms to the University Health Service (UHS) via UHSConnect.<\/p>\n<ul>\n<li>New York State Law and the Ä¢¹½´«Ã½ mandate completion of these forms and all requirements. Required tests and immunizations must be completed and verified by a health practitioner or public health official as specified.<\/li>\n<li>All immunization information must be submitted in English.<\/li>\n<li>Students not in compliance with all requirements by the deadline specified by your program\u00a0will be subject to a late fee and may be subject to additional administrative action.<\/li>\n<li>If you need assistance, contact the UHS Medical Center Office at 585-275-4955.<\/li>\n<\/ul>\n<h4><strong>UNIVERSITY HEALTH SERVICE REQUIREMENTS<\/strong><\/h4>\n<p><strong>Health History and Immunization Requirement \u2013 Must be submitted prior to the start of classes.\u00a0<\/strong>Please read the following documents for more information.<\/p>\n<ol>\n<li><a href=\"https:\/\/www.rochester.edu\/uhs\/primary-care\/mandatory-health-fee\/immunization-requirements-for-students-in-health-professions\/\" target=\"_blank\" rel=\"noopener\">Immunization Requirements for Health Professions Students<\/a><\/li>\n<li><a href=\"https:\/\/www.rochester.edu\/uhs\/wp-content\/uploads\/2021\/05\/Meningococcal-Info-1.pdf\" target=\"_blank\" rel=\"noopener\">Meningococcal Disease Information Sheet<\/a><\/li>\n<li><a href=\"https:\/\/www.rochester.edu\/uhs\/primary-care\/services-for-full-time-students\/\" target=\"_blank\" rel=\"noopener\">Services for Full-Time Students<\/a><\/li>\n<li><a href=\"https:\/\/www.rochester.edu\/uhs\/wp-content\/uploads\/2021\/05\/Notice-of-Privacy-Practices-English.pdf\" target=\"_blank\" rel=\"noopener\">Notice of Privacy Practices<\/a><\/li>\n<li><a href=\"https:\/\/www.rochester.edu\/uhs\/wp-content\/uploads\/2021\/05\/Physical-Examination-Health-Professions.pdf\" target=\"_blank\" rel=\"noopener\">Physical Examination Form<\/a><\/li>\n<\/ol>\n<p><strong>Influenza (Flu) and COVID-19 vaccinations<\/strong> are not required but\u00a0<em>are strongly encouraged<\/em> to keep our patients, colleagues, and community safe. <em><strong>D<\/strong><\/em><b><em>ocumentation<\/em><\/b><em><strong>\u00a0of\u00a0<\/strong><\/em><b><em>your vaccination status<\/em><\/b>\u00a0(whether you got vaccinated or declined )\u00a0<span style=\"text-decoration: underline;\"><b><em>is required<\/em><\/b><\/span> by providing proof of vaccination or declining.<\/p>\n<p>*If you are a Part-Time Health Profession student, you will be charged a $50 Student Health Processing Fee, \u00a0which covers the administrative cost of processing health history forms and ensuring immunization compliance.
